NYSDEC recently updated the 2025 Annual Report Form for this registrant type, and it is now posted on their website.

Tips to Complete and submit the report

This is the second year that third party manure applicators will need toreport. Below are a few tips that will help you successfully complete and submit the report.

  • Download the 2025 Annual Report form
  • Complete the first page using the "Registered Facility (Applicator)" name that you used on your registration and the 'Facility Activity Number" (ex: 42A60021) provided to you by NYSDEC when they notified you of your successful registration. If you are unsure if you are registered or do not know your Facility Activity Number, please reach out to the regional contact listed on the last page of the annual report form.

Section 1 (Facility Information): 

  • Provide and/or update contact information for your business. 
  • If you applied manure on a CAFO farm in 2025, check the 'Yes' box at the bottom of the page and complete the rest of the form. 
  • If you did NOT spread manure on a CAFO farm, check the 'No' box and you only need to complete Sections 1 and 5. Note: if you no longer intend to apply manure on a CAFO and want to relinquish your registration, you need to notify your regional NYSDEC office.

Section 2 (Quantity of Waste):

  • Enter the total gallons of manure applied on CAFO farms for reporting period of January 1, 2025 - December 31, 2025 at the top of the page.
  • For EACH CAFO where manure and/or process wastewater was generated (and you were hired to apply from), you should provide the information requested.
  • If the manure generated at that CAFO was applied only on land operated by that same CAFO you can simply enter the total gallons applied at the farm under Line 1 of 'Receiving CAFO sites receiving waste from this CAFO source' and the name/address can be the same as the generating CAFO.
  • If you applied manure from one CAFO on land operated by more than one  CAFO, you must list each receiving CAFO separately (including the total waste applied at each farm).
  • If you have more than two 'Generating CAFOs' to report, you can either make additional copies of Page 3 and include them with the report OR you can provide your own list, spreadsheet, or table as long as it includes all the same information requested in the boxes on Page 3.

Section 3 (Applicator Equipment)

  • Describe how you calibrate each piece of application equipment to ensure you know you are spreading/recording accurate volumes and rates. Include dates of calibration if known.

Section 4 (Problems)

  • Describe any major problems, complaints, or discharges/spills that happened in 2025 and how they were resolved.
  • Identify if you have had any major procedural or operational changes in your business.

Section 5 (Certification)

  • Read the certification statement and sign/date it if you agree with the description/terms of that statement. 
  • If you have any attachments (additional pages or your own spreadsheets/lists/tables), check the 'yes' box and list the attachments included.

SUBMIT the annual report and any attachment (email is preferred) by March 1 to: 

  •  OrganicsAnnualReports@dec.ny.gov (or New York State Department of Environmental Conservation, Bureau of Waste Reduction and Recycling - Annual Report, 625 Broadway - 9th Floor, Albany, New York 12233-7253.)
  • AND to the appropriate Regional Office (contacts listed on the last page of the annual report form.
